The chapter is requesting your input on the survey below, the information from which will be used to help organize and structure our chapter. There is an option for you to submit your name along with your survey.
If you would be open to a follow up conversation about your answers, please submit your name in the survey and a Chapter Executive Board member may reach out to follow-up with you.
Though the board may follow up with you individually, the EWB-CHPC will not publish personal information when sharing the tabulated responses.
